A form is only part of what a state requires. Before filing, read the state's page for the deadline, where the form goes and anything it must include.

  • Declaration of Paternity Registry form

    The form is not posted online. Iowa's registry rule says it is available from the state registrar of vital records or a county registrar (the county recorder), and the HHS guide says the same. He can ask for it at any county recorder's office, or from the state Office of Vital Records at 515-281-4944. He signs it before a notary and sends it to the state Office of Vital Records at the address shown; if the form he receives prints different filing instructions, he follows the form.

  • Notice of Objection, the pre-captioned form set out in Md. Rule 9-105

    No court website publishes this form. Rule 9-105(c)(3) requires the show-cause order to be served with two copies of a pre-captioned notice of objection form, so the form comes with the order served on him, already showing the court and the case. The notice may state his reasons for objecting and ask the court to appoint a lawyer for him, and it may come with a request for access to the case records (Rule 9-107(a)). He files it with the clerk of the circuit court named in the order. A father notified by publication instead gets, in the notice, the clerk's office address where a copy of the show-cause order can be obtained.

  • 04AN019E (DCFS-21-B), Form Enclosed with Notice of Plan for Adoption

    He returns the copy enclosed with the Notice of Plan for Adoption served on him, which the statute requires to be preaddressed, to the OKDHS Paternity Registry, and sends a copy to the attorney or agency that served him. OKDHS also publishes the blank form; it says to follow the filing instructions in the notice. The form counts if the registry or that attorney or agency receives it within 30 days of service.
